Pathways Home 4 is an opportunity for individuals with 20 to 270 days left to serve on their sentence. This opportunity will help to enhance their job skills and receive assistance transitioning back into the community.

Who is eligible?

Anyone who is 18 years or older who is in a non-Federal institution or on home detention, legally eligible to work in the United States and has 20 to 180 days left on their sentence.

What services are offered pre-release?

Anyone who is eligible will be offered guidance on developing an Individual Development Plan, job preparation and career exploration, monthly meetings with the career coach, and the Making It Work Program. The Making It Work Program is facilitated by a career coach it involves 10- one in a half-hour sessions that will teach soft skills to help reintegration into the workforce as well as pre-release training opportunities.

What services are offered post-release?

Within 72 hours of discharge, the career coach will meet and review their Individual Development Plan, make referrals to legal assistance, if applicable, help with social services to address reentry barriers, occupation training (apprenticeship, transitional jobs, and upskilling opportunities), and ensuring access to housing and food. These follow up services will be maintained for 12 months and will include monthly contact with the case worker.
